Who We Are

Horizons of Hope is an emerging nonprofit dedicated to supporting people with intellectual and developmental disabilities (IDD) who are on California’s Self-Determination Program (SDP) and transitioning or emerging into adulthood. Currently we are focused on serving people in the Inland Valley.  If you’re not yet on the SDP, we can help guide you through that process too.

We don’t provide direct services. Instead, we support the people who do — the personal assistants, aides, and coaches who show up every day for the individuals they serve. We help staff build structured daily schedules and track progress toward each person’s goals, whether those are independent living skills, personal milestones, or behavioral growth. That creates a shared, transparent record for individuals, families, and staff — showing that the work on the Person Centered Plan is actually happening.

We also help individuals and families who are just getting started find the right staff if needed and once we get to know someone, we connect them with service providers,  jobs, volunteer opportunities, and resources that are a real fit for who they are. And because community matters, we work to build genuine connection among the people in our program.

Our role is to support, connect, and resource — so that staff feel equipped, individuals feel seen, and families don’t have to figure it all out alone.

Tailored
Activities

Every person has unique gifts, interests, and abilities — and their schedule and activities should reflect that. We work with individuals, their families, and their staff to help identify and build programs that are a true fit, whether that's creative arts, sports, skill-building, or something else entirely. The goal is a life that feels purposeful and engaging, built around who they are.

Empowering Staff

We provide personal assistants, aides, and coaches with the guidance, training, and resources they need to do their jobs with confidence, consistency, and a genuine sense of purpose. With structured schedules and clear goals in place, staff always know what to do and what they're working toward — giving families peace of mind that their loved one is in good hands, engaged, and making progress, without the worry of having to oversee every detail themselves.

Jobs, Volunteering & Goal-Driven Resources

We help connect individuals with employment opportunities, volunteer experiences, and community resources that align with each person's Person Centered Plan and long-term goals.

Building Community

As our program grows, we hope to build a vibrant community where individuals can meet, connect, and build real relationships with others who share similar experiences. Because belonging matters — and no one should have to navigate this journey feeling alone.

  • Support staff — personal assistants, aides, and coaches — with the resources, structure, and encouragement they need to provide high-quality, consistent care.
  • Connect individuals with services, activities, jobs, and volunteer opportunities that bring their Person Centered Plan to life.
  • Build a community where participants know each other, support one another, and feel they truly belong.
  • Serve as a trusted resource for families and staff navigating the SDP, helping them find what they need without having to figure it all out alone.

Horizons of Hope’s role is to support, connect, and resource — not to replace the individualized services already in place.

A Journey of Hope

Too many autistic adults are turned away from programs that are there to support them. Cole's experience is one many families of people with IDD know all too well.

My son, Cole, was one of them. We were met with rejection after rejection, some programs even refused to meet him before deciding he wasn’t the right fit. One program went so far as to say that if he had an instance, they would call both the parents and the police. Whoever got there first would take him. That moment changed everything.
