About Horizons of Hope

Horizons of Hope is a nonprofit organization dedicated to supporting people with intellectual and developmental disabilities (IDD) who are on the Self-Determination Program (SDP) and transitioning or emerging into adulthood. If you or your loved one isn’t yet on the SDP, we can help guide you through that process too.

We believe that when the right support is in place — for both the individual and the people who serve them — life becomes fuller, more purposeful, and full of real possibility. That’s why our focus isn’t on providing direct services, but on strengthening everything around them: the staff who show up every day, the schedules that bring structure and accountability, the community that reduces isolation, and the connections that turn goals into action.

We walk alongside individuals, their families, and their staff — not to take over, but to make sure no one has to figure it all out alone.

Your Questions, Answered

We primarily serve people with intellectual and developmental disabilities (IDD) who are on the Self-Determination Program (SDP) and who are transitioning or emerging into adulthood. While this is our focus, we welcome anyone who could benefit from what we offer. And if you’re not yet on the SDP but think it might be the right fit, we’re happy to help guide you through the process of getting there.

No. Horizons of Hope does not provide direct services — and this is intentional. Providing direct services would create a conflict of interest with our role as a support and resource organization. Instead, we focus on strengthening the support that’s already in place, or helping you find and build that support if it isn’t yet.

We support, connect, and resource. Here’s what that looks like in practice:

We support staff. Personal assistants, aides, and coaches are the heartbeat of this community — but the job can be overwhelming without the right tools. We provide staff with guidance, training, and resources, and we help them build structured, individualized daily schedules so they always know what to do and what they’re working toward. We also help staff track progress toward each individual’s goals — whether those are independent living skills, personal milestones, or behavioral growth — so there’s a clear, shared record for the individual, their family, and their team.

We help build your team. If you don’t yet have staff or services in place, we can help. We’ll work with you to understand your needs and connect you with qualified personal assistants, aides, and coaches so you can build a strong support team from the start.

We connect individuals to the right opportunities. Once we get to know someone — their gifts, interests, and goals — we help them and their staff find activities, jobs, volunteer opportunities, and community resources that are a genuine fit. Everything we do is guided by the individual’s Person Centered Plan, so connections are meaningful, not just convenient.

We build community. We bring together individuals in our program so they can meet, connect, and build real relationships with others who share similar experiences. Because belonging matters.

One of the most practical ways we support staff is by helping them build a consistent, structured daily schedule — customized to the individual’s needs, goals, and routine. Structure creates accountability and reduces stress for everyone involved.

We also help staff track progress toward the individual’s goals over time, creating an ongoing record that the individual, their family, and their staff can all see. This means there’s always a clear picture of the real progress being made — and tangible evidence that the goals outlined in the Person Centered Plan are being actively worked on, not just written down. All of this is made possible through an app developed exclusively for Horizons of Hope, designed to organize goal data, record progress, and keep everyone accountable every step of the way.

Horizons of Hope is not a day program. We don’t operate a facility or provide a place for individuals to go during the day. Instead, we work directly with individuals, their families, and their staff wherever they are — helping to build better structure, stronger connections, and more meaningful opportunities within the life they’re already living.

The Self-Determination Program is a California program that gives individuals with developmental disabilities and their families more control over the services and supports they receive. Rather than being assigned services through a regional center, participants manage their own budget and hire the staff and services that best meet their individual goals. It’s designed to give individuals more choice, flexibility, and independence.

If you’re not currently on the SDP but are interested, we can help you understand the process and guide you through the steps to get there.

That’s great — and we can still be a valuable resource. We work with individuals and families who are at any stage, whether they’re just getting started or already have a full team in place. If things are working well, we can help make them even better. If there are gaps — in structure, accountability, community, or opportunities — we can help fill them.
